Special Education Credential Application Checklist

Contact the SoE Student Services Office if you have any questions. Process is subject to change.

	Step 1 Applications Complete CAL STATE Apply Application NOTE: Candidates who are currently a Post Bac student and are attending SSU to complete credential program pre-requisite courses or who are enrolled in a Stateside MA program do NOT need to complete Cal State. Contact the Credentials Office Contact us at credentials.office@sonoma.edu for specialized instructions on applying to the program. Refer to Preliminary Credential Program Application Overview
	Step 2 Statement of Purpose Refer to Preliminary Credential Program Application Statement of Purpose
	Step 3 Basic Skills Requirement Proof of exam registration or Copy of official score report or Basic Skills Self Report Form Refer to Preliminary Credential Program Application Basic Skills Requirement
	Step 4 CSET Exam/Subject Matter Waiver Proof of exam registration or subject matter waiver in progress Copy of official exam report or copy of letter verifying completion of waiver Refer to Preliminary Credential Program Application Subject Matter Competency
	Step 5 Verification of Pre-Program Field Experience Field Experience Form Refer to Preliminary Credential Program Application Pre-Program Field Experience
	Step 6 Transcripts from each institution attended One set of OFFICIAL transcripts sent to SSU's Admissions & Records Office Candidates do NOT need to provide an official set of SSU Transcripts. Please attach an unofficial set of SSU transcripts to your application. Refer to Preliminary Credential Program Application Transcripts
	Step 7 Letters of Recommendation Two professional letters of recommendation required Refer to Preliminary Credential Program Application Letters of Recommendation
	Step 8 Certificate of Clearance (CofC) 8A: Get Fingerprinted: Complete Live Scan Form. Bring completed form to a live scan operator. Keep copy of form once you have been fingerprinted in order to complete following step. 8B: Apply for CofC with CTC. Submit CofC, or verification that CofC is in process of being issued.
